im not but matt, but i can answer, you must go to the sheriffs department at the county court house. I believe the hours are between 8am and 430pm. They have forms there, or you can fill it out and print it from here. http://co.armstrong.pa.us/files/sheriff/AC_LTC_APP.pdf. The fee may have gone up to $25, so I would recommend that you take in a blank check and make it out there. You will also need to take a state ID (DL, or ID card) and two references, which will be listed on the form. They will take your picture there, and is not required to bring one with you. It normally is compleated in less than half an hour and you will walk out with your new LTCF.

The only part of that I'll challenge is 4:30, if the courthouse is slow everyone starts sneaking out around 4:00 so be early lol.
