Lot's of people misuse steel targets. There are minimum standoff distances and ammunition limitations that aren't always followed. One 5.56 on a pock marked soft steel target at a close distance can be a big problem. I wasn't there and can't say for certain what happened, but when the article mentions "ricochet", I have to wonder.

That's usually a pretty busy range. My club trains our retrievers across the street from it at the Winslow Wildlife Management Preserve.
