I've noticed depending on the type of projectile and how you are angled to the paper that may happen. Sometimes I get clean circles, sometimes it looks like that.

When I looked at the target, I saw round holes with paper torn, a keyhole shot would be clearly defined and not have a round hole in the center of the hit.
