lehigh county cc permit application

Hi, I have an expired cc permit and I was looking to download an application online. I went on the county sheriffs web site but couldn't find anything. Anyone have any info, I really didn't want to have to drive over to get one! Thanks for your help!

I believe this is the most recent version of the form which is supposed to be used statewide: http://www.montcopa.org/DocumentCenter/View/3880

Print it as one page front and back. Depending on your printer you may want to let it dry briefly before running the back through for the second page, so it doesn't smear.
