depression can be linked to chronic pain, and since disc/back pain is often chronic her question makes sense as a general screen.

Might feel good to say it but probably noted on your permanent medical record...

This question about harming oneself has been asked for years by my local VAMC (VA Med Clincic) when reording meds.

A couple of years ago one of their telenurses checked the wrong box when I had answered NO to "Do you ever feel like harming yourself well taking (specific) medicine?"

Took me a couple of days and some time on the phone to get the paperwork and computer data corrected plus I reported the incident to the VA Regional Office for good measure.
