Grip safeties on XDs causing problems?
I've read a couple comments that the grip safeties on XDs can malfunction and lock up the gun. Does anyone know of an incident of this actually happening?

IMO it's a mis-statement.
Some people don't understand the weapon. The grip safety must be fully depressed for the pistol to function. IMO it's most likely an issue of operator error.

No problems on my end, nor have I ever heard of any issue caused by this.

The only issue I've heard of was a guy who clearly needed to just practice more would obtain such a sloppy grip from the holster while trying his mall ninja "quick draw" that he would sometimes not disengage the grip safety.
With a grip that sloppy, he was a danger to himself and others and everyone told him so. I don't know if it ever sunk in to his thick stubborn head.
To answer your question, I have 3 different XDs, have, on one occasion that comes to mind, fired over the course of 2 days well over a thousand rounds through one of them (XD45 compact), without cleaning or maintenance of any kind (no time to do so) with zero,zip, nada malfunctions. And I have never even heard of an issue with the XD fire control system except for the above noted operator error. Whatever that's worth.

I have heard but couldn't duplicate with my XD45 that when you assemble it after cleaning there is a way to jam the slide if the grip safety is not fully depressed. with this pistol the grip safety must be depressed to work the slide in that manner. There is no mechanical device that is completely fool proof somewhere sometime someone will find a way to gum up the works. I have heard of one instance where some adventurous soul had fired over 5000 rounds without cleaning and it was still going strong. No, I have no idea what ammo he was using.
