Not much corrective action to take if you assemble the components correctly and the anvil is bad or the primer compound is bad.
I've been checking all of the casings I collected a little at a time. The dents all look ok however on the 1911 this happened to the primers are striking just a little off center. We had 3 different .45 caliber 1911s there at the time. This particular one I put together.

I had my first over the weekend.

It had nothing to do with my powder issue discussed in another thread. This was a 9mm I had loaded some months back. Was having a grand time firing and then "pop" and some smoke.

Although I had never experienced a squib or seen a video of a squib, my brain seemed to instinctively say "that's a problem". I stopped, dropped the mag, cleared the chamber, where I found a very dirty un-ejected case. Stuck a chamber flag in and could feel the slug stuck in the barrel. Took it home and pounded it out with a wooden dowel.

Kinda shocking when it happened, but I feel better knowing that I will recognize it if/when it happens again.
