Proof of residency.
Bullshit that it is, PPD requires two proofs of residency for LTCF. I have one, which is my voter registration card, but nothing else that qualifies. Any ideas?

Do they really think anyone in their right mind would put up with the GPU that didn't have to? Really?? Delaware County is bad enough, but there is no way in #&!! I would try to get my LTCF thru PPD unless I had no other choice.
