Another Invasive healthcare form
I know this issue has been brought up before but I wanted to share my response.
I had to go to a new doctor recently due to the fact that my old doctor retired. Upon arrival I received a packet of information that had to be filled out and was surprised to find this questionnaire that I found to be invasive.
The interesting part is when the nurse reviewed my information she saw my statement and said “I don’t blame you”.
The doctor did not ask or seem to even care about that part of the form, kind of makes me wonder why they even have those questions on it.Last edited by SammyL; June 18th, 2012 at 12:10 PM. Reason: Spelling error

I believe the word you are looking for is "invasive".
I have been to at least 1/2 dozen new doctor's offices in the last 6 years or so, I keep looking for these questionnaires. They all have some variant, but I have yet to see anything about firearms. I have always wondered how to respond. Do you ignore it? If you answer all other questions but leave gun-related questions blank. you give a pretty good hint. Same with answering "none of your business." None gun owners will likely gleefully answer those questions, anxious to demonstrate to their overlords what superior subjects they are.
I suppose the best tactic is to simply lie. Or tell the truth?
What has everyone done with these?

I would leave all questions that aren't directly related to a medical issue blank.

I wonder whether these kinds of forms are a reflection of the insurance company,instead of the doctor? I have only had 3 medical insurance companies in the last 30 years or so.
I always get the medical history forms, but never the "lifestyle" forms. And I look for them.
In addition to them being invasive, I also find them condescending and insulting.While many claim to support the right, precious few support the practice.

You do understand that you actually answered YES to the gun question?
Either answer all or none (non medical) or they will know by which you did not answer.

^^^^^ That's always been the way I do it. If I don't think the questions are pertinent, i skip them. This hold for many different places. Just because questions are being asked on a form, does not mean I have to answer them. I've never had someone say "you did not answer these questions, go away".

This is another in the long line of incredible reversals that have happened over the past 1/2 century....
You are hiring the Doctor --- not the other way around....
I provide information only related to what I hope to get from the doctor's services THAT I WANT.... Satisfying his agenda or the pharmaceutical agenda or the government agenda is not my goal...
I refused to have my picture taken (why they even needed it I'll never know - it was procedure??? yea, right...) at the last visit to a specialist...
'1984' is looming larger and larger all the time....

I OC'd to my last doctor visit (typical sinus infection crap, just one of the local 'emergency' care places). The nurse and doctor both commented but didn't really seem to mind too much. I've yet to have the question on their forms... I think on my primary physician form they did have that question, but I can't recall if I answered it or not... They also asked stress type of questions...like, have you moved? Lost your job recently? Had a death in the family?... a couple others... I wrote-in "All of the above", no psych call so far... :-P Our new pediatrician visit is coming up, we'll see if they have it on there...
