There is some information here about the process. Read all the way down. There are also other forms at the bottom in pdf format. From what I can tell, you will have to write out all of the pertinent information and provide a copy of the receipt so that whoever is assigned to read the petition has all information clearly in front of them.

I am not a lawyer and I have never done this. This is where I would start. It might be worth paying for an hour of GL's time to get the right info.
