Does anyone know of a shop in the vicinity of Pittsburgh that does custom laser cut high density foam inserts?

I dunno about laser cutting, but a waterjet shop should be able to help you out.
Laser's not a great tool for foam: too much heat.
should be some job shops here

I was hoping more to get a recommendation from other forum members in the region as to a company they have worked with in the past and had good experiences with.
