Be sure to check your charges!
This week, two college buddies came in to town for a concert and I wanted to show them a good time at the range.
Shot all of my pistols with no problem and then took them to the rifle range.... the pictures tell the full story.. We were about 500 rounds into my AR when my friend was shooting and we heard an unusual sound. I'm not sure if one round was undercharge, and backed up the barrel of if one round was double charged.
The bolt, barrel, and upper are toast. It was a PSA upper and 16" barrel. Lesson learned. I pulled my remaining 200 reloads and am in the process of redo-ing them.
cudos to eo-tech, the reddot is still in working order. my upper rail went flying about 10 feed.

Second, when you hear a funny sound, ALWAYS check for squibs.

Yea, I was standing to his side and thought his face was going to be gone. Guns can be replaced. It was only his 2nd time shooting so he didn't know the difference but ultimately my error with the reloads. I'm a bit timid to reload against but will press on.

WOW! check, check, and double check, case length and overall length, if the bullet gets pressed into the rifling excessive pressures will result

First and foremost: I'm glad that neither you nor your buddies were hurt.
How would sending another bullet down the pipe after one is already stuck cause the upper to blow apart? I would think the barrel would have blown since the point of impact of the two bullets would be somewhere in the barrel. I admit I don't know much about it and look forward to other members observations on what my have happened here......
Secondly, you really can't double charge a .223. Loads fill the case pretty full and a double charge would have you spilling powder all over your reloading bench.
Just a few thoughts and again thankfully no one was hurt.
