Rating system for gun shops

Ok, this is what I have come up with so far. I figure we can rate the shops based on this and whatever we want to add or take away from the criteria is up for discussion.
I think a 1-5 rating will work best and give a running average as more and more are filled out over time about that particular shop. I would also like to add a Google map function at some point as Steve suggested and whatever other features we think would make it user friendly.

I think that looks good so far. Do you think you could include a space for hours of operation and a phone number or is that getting to be too much info? I only think of it because alot of smaller shops keep some uncommon hours.
