maybe he thought the grip goes into the holster, and the barrel points forward?

as suggested earlier, he was sitting on a couch, with his pants unbuckled and pulled halfway down to his knees, and she was laying on the couch with her head in his lap
on another note, I've been grumpy lately and think I need more 'hugs'

That's what my wife said.
Hopefully you've caught up with the details that were posted by now.
That was something I considered, but it's a distant second, or third, or fourth, after a greater chance that someone did something STUPID.
Did she maybe snag his gun while she was gittin' jiggy wit it? Some people think it's funny to snatch a gun and toy around with it.
Hard to say really, but I'm betting that more than one person knows what really happened.I called to check my ZIP CODE!....DY-NO-MITE!!!

This. When I carry, and I "drop trough" for "business reasons", the gun defaults to pointing up and forward from my left side. I always pick it up and fold the edge of my pants so that it's pointing downward between my feet, handle away from me.
However, if I left it in the default position and I were being "hugged" by some young lady in that position, it's very likely that any discharge could end up in her chest.
I'm pretty sure that'd be one of the few times I'd remove the gun from my person and leave it within reach...
