It's a joke, of sorts.

If one uses the median as their measure of central tendency and/or average, then by definition there will be 50% of a population who are "below average" and 50% of a population which are "above average".

On a more serious level, it points out that even the doctors who graduated at the bottom of their class still graduated and are therefore doctors who are treating patients. Just because they have a degree doesn't mean they are a good doctor.
