I'm a little confused. I want to connect a 20lb tank and refill a 1lb. How do I do that with only this?

This is what you use to refill a 1lb bottle from a 20lb or larger bottle. I believe that current regulations you're not allowed to refill 1lb bottle because they do not have a over pressure relief valve. Please be careful refilling 1lb bottles.

They are harder to work with and you have a large chance of losing propane because your "valve" is unscrewing it. If you get the one I first posted, you can put the bottle on a scale and weigh it as you fill it so you'll have less chance of over filling. Keep in mind that the tank you're filling from is supposed to be upside down so you get liquid into the 1lb bottle, so the second adapter I posted is difficult to use.Rules are written in the stone,

You use the adapter to screw your 1 pound can to the 20 pound tank then turn everything upside down (this is the hard part). When you unscrew the valve you will hear gas hissing slightly while it is being transferred. Without the bag of ice trick this can take a long time and you won't be able to fully fill the tank. There are many videos of this on Youtube.
I think at one time it was illegal to refill a tank (maybe it still is) but no one cares anymore. It was once difficult to find this type of fitting but now they are everywhere and in major retail stores. One pound tanks do have an overpressure valve but to be safe tare the empty container and only fill it with one pound of propane. I did massively overcharge one tank and I had to release a lot of gas before I wouldn't bring it inside or travel with it. I just screwed the adapter to the can and watched it from a distance as the excess propane vented. When liquid stops coming out and its just gas you are done. Looking back this was probably stupid and if I had to do it again I would hook it up to the stove and let it burn off for a while (outside).
And always use the brass caps when storing indoors. I had new never recharged tanks get stuck and leak.

So that little adapter connects the two, nothing else needed as it shows a hose and regulator on Amazon photos. Like what presses in on the valve on the 1 lb so it is open and can fill?

I am glad you said something because I didn't look at your link. That adapter is the wrong adapter. It looks close but is for converting a 20 pound tank from the POL screw in to the quick detach connector.
You want one like this. It has a screw connector for attaching to the 20 pound tank (reverse threaded) and the female connector for the 1 pound tank.
