Question about Curios & Relic renewal form???

Does any body know how to correctly fill out form 922(g)(5)(b) if you are a US citizen renewing a C&R license?

This form is certification of citizenship but is confusing. If I AM a US citizen by birth do I have to have somebody else attest to that?

No, you put your name, and then your name again.
Reason being if you are certifying your corp name, etc you would ut that first and then your name second.
