Best I could find for you is an instruction sheet that has tips on how to fill one out, but it does show most of the form (at least the October 2008 version of it).

Misleading headline (big surprise).

"Drunk in public" is illegal there whether armed or unarmed. The gun made no difference to the charges. Just more misplaced hype.

Simply being "drunk in public" is not illegal in CA unless.....

The offender is found in any public place under the influence of intoxicating liquor, any drug, controlled substance, toluene, or any combination of any intoxicating liquor, drug, controlled substance, or toluene, in a condition that he or she is unable to exercise care for his or her own safety or the safety of others, or by reason of his or her being under the influence of intoxicating liquor, any drug, controlled substance, toluene, or any combination of any
