Re: Have you trained for malfunctions like this?
Yes! I use a cross pull draw, using my weak side hand to clear my garment. It eliminates that issue from arising for me...

Re: Have you trained for malfunctions like this?
Yes, my undies do sometimes stick out. We wouldn't want to cause our mothers to be embarrassed if they knew we drew on some BG and our undies were showing.
LOL, I am a woman...

Re: Have you trained for malfunctions like this?
Drawing from beneath a cover garment must be practiced. Other than an open front garment, all other draws are two handed. A two handed draw from under a tee shirt requires the support hand to be used to pull the shirt clear of the gun and holster. That action prevents the failure illustrated above.
Some may argue that the support hand is not always available. It could be disabled, missing, or otherwise engaged. When that is the case, use a high thumb under the shirt to clear the bottom over the rear of the slide as the hand moves from stomach to the holster.
Both techniques must be practiced until natural and reflexive.Never underestimate the value of early training.

While I appreciate the discussion threads like this foster, I wish it didn't feel like an advertisement.

A loose garment as such wouldn't be the best. How about a light vest, something a little stiffer to drap away.
