The Motion to Vacate rule was one of the things McCarthy agreed to when the handful of Republicans (lead by Gaetz) were holding out and forced concessions.
For several years prior to this current session, a Motion to Vacate required a majority of the conference to force a priority vote (within 2 days). The concession McCarthy agreed to, in order to win enough votes to become Speaker, allowed for a single member to make the motion, and trigger utmost priority for an up or down vote within 2 days.
In order to be elected to the position, he agreed to change the rules that would more easily allow for his ouster. And Gaetz was the one that forced that rule change, once McCarthy agreed, Gaetz changed his vote to “present”, which I believe put McCarthy over the threshold
