AR15 problems

The rifle I built my niece is doing this on every round. Anybody have any idea whats going on?

Are those each the first round (attempted to be) fired, or are they ending up that way after the first round goes downrange?

Does it fire any rounds at all, or those the result each time a round is "chambered"?

She says the first round fires, the second round jams, and thats what happens when she clears it. Always the second round.

I wish I was up there with her I think I might be able to figure it out.
