    You are correct that the PSP website does not provide a copy of the generic form. I suspect that is because the public source for the form is the sheriff's office of application.
    Note that ChesCo has significantly altered the header of the form and will insist applicants need to fill out "their" form for submission. I went through this recently but was steadfast in responding to their request with something along the lines of, "so are you refusing to accept the official form created by the PSP?" After some back and forth behind the counter and about 10 minutes wasted, which nearly defeated the purpose of stopping by with a pre-printed form in the first place, they finally accepted the one I brought in.

    I reached out to the Sheriff at the time but not surprisingly I never heard back. She was probably too busy skimming the county books.

    I did the same thing when the last revision came out but was not yet adopted by my county. No push-back, but I was ready. With so many counties offering it online, it's pretty easy for anyone to find a "true" unaltered version for use.

    I don't like the idea of the process being taken over completely by PSP, because they're certainly no better, but it would be nice to have accountability that would sting enough to keep Sheriffs in line. I wonder what they think the "U" in the UFA is supposed to mean.

    You're right, they still have those 2 extra pages... However, I'm curious whether they hand those out in person still? And if they still mail out the reference?

    They may or may not have made changes???? County level IT is ridiculously slow to catch up.
