A crazy question... Do you have a rifle spring in a carbine tube?
Did you buy the pistol brace or tube as individual parts or as a kit?

Take out the buffer spring and count the coils.
It should be 38 give or take, if it's over 40 it's a rifle spring.
You need a carbine spring. Just a shot in the dark.
If you are running a free float rail, some gas blocks are made to line up with a small gap away from the shoulder on the barrel where the handguard cap usually sits.
If you have a gas block with set screws on the bottom, one set screw hole will line up with the gas hole in the block. You can rotate it until the hole lines up with the hole in the barrel to see if you need a gap, usually you don't but I have had it happen on one of my builds years ago, half the hole was covered. If the barrel is dimpled from the factory then don't worry about the gas block alignment.
What does the brass look like, are they as shiny as new or dull and pitted?
Could be a rough chamber.
When the brass does eject, is it out in front, behind or out to the side?
You say the brass just falls close to the rifle, is the ejector plunger stuck or weak?
When it doesn't cycle does it eject the spent case then not feed another round?
Do you get a stove pipe, failure to extract, failure to eject or a double feed? What happens?
