Default made a mistake on LTCF app/interview(Philadelphia pa)

Hi and Good Morning and Good day to all forum supporters.

I filled out my application recently for my LTCF in Philadelphia Pa I am a previous LTCF holder from another county but now reside in Philadelphia and needed to renew apply for a new permit in Philadelphia cause they said its not a renewal because I never had a Phila LTCF before(even tho Phila is in the same state where the county resides that I had a previous LTCF but whatever thats what they said)

MY question applies to a mistake I made when they interviewed me that I realized after the fact.

They asked me for previous addresses besides my current Phila address that I LIVED at.

I briefly used a PMB as an address while in process of moving between one address and my current in philly. Both of which I gave except the PMB(personal mailbox rental) because I never lived at the PMB and thats why I did not give it cause they asked for addresses where I lived. It is however going to come up as an address for me most likely during there investigation because my credit cards were sent there.

They (the interviewing officer) also forced me to give my SSN even tho it said optional and I was saying I did not wnt to put it on the app sheet but the officer insisted I do! I am concerened because it says your info can go public on this app on a website belonging to the city, is this correct?

Any help and answers will be greatly appreciated.

I don't think any advise given would be helpful. Philadelphia does it their way even though they know the state has specific rules about the ltcf. They have not been held accountable yet so things remain the same. I also think it depends on the individual dealing with your application.

I wish you good luck and hope you do not have to jump through any more hoops to exercise a right.

Compared to the usual stuff we get here this does not sound major.
Philly likes to give people a hard time so it's hard to say on this one.

If it does come up, explain it just as you did above. If it does not, don't worry about it. Don't try to change things, that will bring unwanted attention to you and your application which will not end well. Good luck and let us know how it comes out.

This was my first thought as well. The only difference is that I don't think they will ask for a clarification, I think they would just deny. You would then need to appeal it. I do not think that will happen though.

I didn't see anywhere on the single page form where it asks for previous addresses.

Intentionally making false or misleading statements on the application form!
