I handed it in blank. When the office lady said I needed to fill it out, I informed her that wasn't going to happen, because that form was illegal, the only thing that needed filled out was the one page form from PSP. I got my stuff after about 20 minutes or so after I handed it in. I overheard her mention to someone in the office (who was obviously being trained) that stated references are only contacted if there is some problem with the PICS check.
Great job for sticking up for yourself.

    Final Update for DelCo:

Renewal Application turned in, fee paid, photo and signature taken: 2/12

Approval Letter postmarked: 2/22 (10 days)

Approval Letter received: 2/24 (2 days in the mail, 12 days total)

First day the office is open after letter received: 2/26 (14 days total)

Way better than the 46 days from application to approval 5 years ago, but still not same day...

Honestly I expected it to be rejected and to end up calling a lawyer. I made sure I had the number on hand.

This past weekend I happened to talk to one of the deputies, who is in my army unit. Seem some school hysteria around here, two gets have been arrested for threats and the like, we talked about work stuff. Eventually I mentioned the extra form and that it was very illegal, and told him what happened. I noted that since I pay taxes in Juniata county I would dislike seeing the Department sued, but it is only a matter of time with that illegal form. He said he'd talk to his boss about it, and thought maybe he didn't know about it.

    So we'll see.
