    Yep , I've had , and heard tell of dozens more incidents of mostly extended safeties working 'off' , sometimes with bad results. That's why I prefer a holster with a thumbreak strap the fits tightly on the back of the slide with the hammer back , and I also prefer a standard thumb safety.
    I removed the factory ambi safety from my 1911 before firing a single round and replaced it with a single-side extended piece from Wilson Combat. When I added a matching 1911 to the stable (one to shoot, one to carry) I did the same. I've carried one or the other in the same hybrid holster pretty much daily for 4 years with no indication that the safety even thought about disengaging.

    Obviously, YMMV...

    Not too long ago while helping my wife's sister with a leaking ac unit, I was bent over cleaning up the water, when I hear her sister scream "your carrying a gun! " my immediate reply was "why aren't you?"...she lives in Easton

    Back in the Eighties I owned a mini mart. Working late one night, partner and I decided to take a break and go outside to the parking lot and play catch with a Frisbee. As it happens, there was a tanker delivering gasoline at the time. We didn't think anything of it and were blowing off some steam. As my partner ran to catch an errant throw his nickle plated Airweight Chief's Special popped out of his ankle holster and went skittering across the parking lot in front of the tanker.

    My 1911 had an extended safety on it when purchased.
    That got replaced with a stock one immediately after unholstering it one time to find it had got bumped off.
    Scared the crap outta me.

    While at my gunsmith's place, I had him firm it up so it takes an effort to snick off the safety.

    Installed an extended slide release at the same time so I don't have to shift my grip after inserting a new mag.

    20+ years of carrying (Model 60 or 92FS) never had an "incident"...about half the time carrying either in a cheap Uncle Mike's or OWB Galco & Blackhawk! leather...
