Are the two separate Ref. sheets still required in Philly?

Has there been any change to that requirement since the recent lawsuit victory? I still see it listed on the Phila. LTCF application site.

There has never been a legal requirement to fill out anything other than the state police issued, standardized (1) page LTCF application that every county in the state uses.

Yes, but as was recently witnessed, it took a lawsuit to make them (if indeed they do) comply. The Philly site still says that you will be denied if you don't have the two ref. sheets filled out.

Let them deny you for not filling out an illegal form
Law requires a certified letter stating reason for denial
Sue them with letter as evidence

A lawyer's advice if that's the case:

This is a common trick in departments that don't like to obey the law. If they don't accept the application then they don't have to reject it and comply with the certified letter, statement of specific reason, and appeal-by-right provision.

I'd take the application in with a printout of the PA law, demand that they accept it, and write down the name of the official who refuses, along with the date and time. Then treat it as a constructive denial and go to L&I with an appeal.

I have a few things working in my favor. I work in show biz...Rock n Roll actually...and have a SS clearance as I need it to work the shows in D. C.
