I can remember waaaaaay back, they used to have you list one of your handguns on the form and that was shown on the hand typed paper ID card.

Just a county form. Pretty basic and I wasn't even sure they truly needed it for the check vs. to make their lives easier for the background check. See the last page of this PDF: http://www.co.lancaster.pa.us/sherif...PPLICATION.pdf
It's older but that form is still apparently asked for. Not a big deal IMO. This would never have happened in my former state of residence.

I'm in the process of renewing my LTCF in Monroe and I'm trying to get some useful information to show the Sheriff's Office. This thread is really great at gathering info on the process, and I think it should be stickied so that we can get a greater amount of data from more counties.
I think it's useful to note that of the 67(?) counties, we have data that 16 will issue on the spot and 2 more that are next day, and that is without hearing from a large number of counties. Maybe showing the counties that like to take the full 45 days how awful they are compared to others will make them care to improve the process.
As for extra requirements, Monroe County refused to accept my application without the "required" tax form. They take your picture and $6 when you drop off the application, and the other $14 when you return to pick it up. They wouldn't start this process for me as I did not have all the forms they wanted, and the wouldn't acknowledge me signing the form in front of them. I'm going back soon armed with more information (and a copy of the UFA) to see what I can accomplish.

I stopped in the DelCo sheriff's office a month after I submitted my application because I was in the neighborhood and thought I would check on the progress. I mentioned Lackawana county issuing same day. I recall one of the women behind the counter saying something like "well, we like to take the time to do them right".
IOW, don't think you can 'shame' them into doing it 'better'...
