My hope is that I can definitely say that Philadelphia is the only county with "additional" requirements.

Also if/when the bill is passed that allows application for ltcf in an adjacent county(SB1071), this will help you choose.

Require additional documents aside from state issued ID to prove county/state residency?: Yes. Required a damn TAX FORM, called a Berkheimer, also known as the 1% earned income form.

Require fingerprinting for application?:No

Required to bring 2"x2" passport type photos?: No, sheriffs office takes the picture

Issued on-the-spot and/or same day?: No

Issued within 45 calendar day? Yes. Took every bit of the 45 days.

I recently got ahold of the local Sheriff. Asked what normal turn around times are these days. He told me that if there isn't a large number of applications, or it isn't done late in the day. It is done on the spot. Some are taking a day or two.
