It sounds as though your slide stop is installed incorrectly. Did you install an aftermarket one? Either way, the TOP PIN needs to be installed first on reassembly to insure the curved piece of spring steel on the slide stop goes UNDER it ensuring that the slide stop works properly. To test it, take your slide off and lift up on the slide stop. It should spring downward.

You may have a bad mag spring or you be be riding the slide stop when shooting the gun.

If you push up on the slide stop, of course it will stay open for every shot. You also may be manipulating the slide stop with your thumbs while you shoot.

And a good way to check if you thumbs are hitting the slide stop when shooting is to shoot one handed. If it locks back on last round shooting one handed, adjust your grip.

Also good to try different magazines.

Could be a couple of things as mentioned, insert an empty magazine and attempt to lock the slide back manually, if this works you are more than likely riding the slide release with your grip. To know for sure shoot the pistol left hand only I am assuming your right hand dominant ? If that solves your problem look no further, you will just have to modify your grip to avoid riding the slide release.

If the above does not work, remove the slide and lift up on the slide release it should snap back down under its spring pressure. If it does not then more than likely it is installed incorrectly. The locking block pin is the first pin out and first pin back in during disassembly/reassembly, the locking block pin also retains the slide release spring. Make sure it is installed correctly there are an awful lot of disassembly videos all over the internet.

    Should solve your problems

If you check all of the above and still have trouble, make sure the mag follower is coming all the way up. It could be hanging up near the top of the mag.
BCM and Glock...for a bigger pile of 'cold dead hands' brass.

I would go with you're probably riding the slide stop lever during recoil and just not allowing the slide to lock open.

My glock is totally stock, and am pretty sure my thumb is not doing it,it doesn't lock back with an empty mag in, I have to push up on the lock.
