LTCF process in Lackawanna Co

Can someone give me the step by step process to obtain a PA LTCF.
I live in Lackawanna Co.

Go to the court house with your DL. Go to the Sheriffs office - see Deputy at window. Fill out 1 page form. Hand form to Deputy. Deputy takes your picture and runs your background. Pay Deputy. Say "thank you". Leave with LTCF.

Hows that for step by step - takes 20mins.

It was $25 and I paid cash.

Make sure you have the names, addresses and phone #s of your 2 references with you.

Went a few days ago. DL and 2 references(name, address, phone number).
$20 and probably 7 minutes.
Everyone there seemed real polite. Easy as can be.

Sure, they make the process easy compared to other horror stories I hear about on these boards, but I do recall them insisting that my references had to reside within the county. One of the individuals I was going to use lived 3 blocks into Luzerne Co.!
