If anyone is interested, attached is a copy of SP 4-127 (10-2008) . It's the latest one I could find. It was a few years newer than the copy my local PD gave me.
I filled it out prior to going to the Sheriffs Dept. Saved a little time.

Yes, that is the very latest one. Unfortunately some sheriffs still use an older one and still issue the older paper-only LTCF that still has the space for your SS # and does not have the License Verification Phone Number on it :rolleyes:
We should have a place on the forum to file this and other documents for easy access.

I've been looking for a "latest" LTCF app. since last summer! I even tried to track down who's in charge of creating the darn things in the PSP (wound up spinning my wheels furiously there). The newest I could find (admittedly, I did not exactly devote *all* my time to this - and I, basically, 'gave up' last fall) was from 2006.
Where ever did you find this?

If you check the sheriff's websites you'll find this latest version on some of them now, including as mentioned above, Westmoreland. Of course some may not have the county blank.
I've never been able to come across anywhere, like on the PSP website under firearms, the "official" copy or even the PSP guidelines for the questions on the app. But as long as you use the latest dated one available, that will be it.
The UFA does not actually state which questions should be asked on the app, which you think it would. It does state exactly what all is to appear on the license, but not on the application.
Right now the most current application is dated 10-2008 (October 2008).
So I was reviewing the most recent PDF and nowhere on the application does it state that references MUST be from the same county as the applicant or anything about providing your local 1% Berkheimer statement. Sheriff Martin in Monroe County has since reverted to the paper only LTCF (no more plastic card) but still REQUIRES references to be full time residents of Monroe County etc. How does someone go about getting him to obey the law. In fact the PSP form doesn't even say references have to be PA residents at all.
