Yikes. That is one reason I never got into reloading, seems even by an experienced loader simple mistakes happen with violent results. Poor gun, glad the guy wasn't hurt.

Wow that sucks, yes mistakes happen but i bet he'll be checking all his cases before he seats a bullet again, thats why all my loads are made on a single stage press.

I’m not so sure this was a double load, first, the bullet is pretty full when you load, second the lead did not make it out, this looks more like some one shooting very fast, and there was a fail to fire, slow primer possibly, the lead tried to get out of the station after the barrel. Or the cylinder did not lock up and shot a few seconds short or late of the lock
The other possible reason was a bent crane where the bullet had an angle to make to get out. Either way, the reason I say this is the lead in the picture, it is not a piece of lead that went through the barrel. ![]() Notice this is 3 shell casings, 2 from the explosion, ripped apart, one piece of lead and a good casing. There would not be more then one damaged casing had the explosion happened in front of the barrel this explosion happened when the bullet was not in front of the barrel, then the flash hit the back of the cylinder and flashed the second bullet to also explode also not in locked position. IE one at 11 o'clock one at 1 o'clock to give the idea. thus breaking the cylinder upwards and pushing the backstrap up. the lead is sheared out the side and this is what you get.
