That's not true in general. Guantanamo Bay is under US control by treaty. Usually the bases aren't considered American territory. For example, RAF Lakenheath in England is "RAF Lakenheath is the largest U.S. Air Force-operated base in England". US operated, but not owned.

WRT McCain. He was born IIRC at Ft. Clayton in the Panama Canal Zone. At the time the Panama Canal Zone (and up through 1999) was a genuine U.S. Territory, just like Guam, Puerto Rico or American Samoa.

The children of U.S. service members born while the "family" is serving overseas (don't forget accompanied tours of duty are allowed in Europe and a few other places) are U.S. citizens under certain circumstances and the birth must be registered with the U.S. Consulate overseas.
