There are a number of reasons why that could happen. Usualy it is caused by the bullet catching the barrel extension when feeding. HP ammo tends to do it more often esspecialy if your rifle doesn't have the M4 feed ramps. I am not familier with Fiochi ammo's crimp either, if the crimp is lite or not at all then they will set back easier.
I doubt there is anything wrong with the rifle. It is probably just the ammo. JMHO, Rich W. |

+1 to what warner said, it could have been the angle that the bolt picked up the cartridge at and it snagged a bit on the feed ramps. A light crimp could also just not hold the bullet securely enough, or a combination of the two. I wouldn't consider Fiochi cheap ammunition, it was probably just a round that slipped past quality control, although I'm not familiar with their crimp either. There is also the chance that the overall length of the cartridge was slightly out of spec and that could be another reason why it might have snagged or drug across the feed ramp.
