Default LTCF form question?

Does anyone know of a place online that one can "print" out an application form?

(Our county sheriff website appears to be down, and apparently they're not very good at returning phone calls either.)

So I was hoping someone else might have a link to the form. I could fill out. And save some time. (Or the possible headache of "well we're currently out of that application form".)

Go to http://www.sheriffofcolumbiacounty.c...biaCCWform.pdf you can fill it out online, take out the Columbia at the top and fill in your county, the only problem on it is you will have to fill the 2 references phone numbers by hand, the first one works well, but for some reason, when you fill in the second it defaults the first one to that same sesong number.
You can not save it to disk, but you can print it. Print a few copies that way you can correct if needed. You can save the blank PDF to your computer, just cant save the filled PDF.

Frenchy...a thought occured to me...although one would hope that using the Columbia form and entering in "your" county would be acceptable, don't you think that some counties may be touchy about this and not accept that form and want to use their own county's form? Not right, I know, but being how each county seems to have some different quirk about how they handle these applications, maybe something to keep in mind.

As we have seen in the past some counties use forms with older dates on them. Wonder if the Columbia form with the date of 5-2006 is the latest one available?

Anyway, hopes this works out for the person who asked the question.

Actually, the form I posted is the only accepted form, although some Sheriff's offices have forms with their logo on it or other types of recognition, I am pretty sure they appreciate the info being typed on rather then printed, (If you saw my writing you would understand). Personally, If i worked for the Sheriff's office and saw the form that conforms to Pennsylvania LTCF even if our department had its own form... I would not question it, could get my department in trouble for tampering forms.
