I looks like Richardson is already in full position while Hillary and the other woman are contorted out of normal posture, possibly looking at something emerging (performer coming to the stage perhaps?), I doubt they're standing like that for the duration of the performance. If the anthem was already in progress, why wouldn't they both turn towards whatever it is they're looking at (which is in a different direction that Richardson's focus) and plant themselves into a normal standing posture?

It is entirely conceivable that Richardson is oriented towards the flag because he noticed the camera and is hamming it up, while the two women get into position prior to the anthem being performed and the two of them and Obama are watching for the performer or some other unrelated event before the anthem started. We have to remember that a picture is a freeze-frame of a single moment in time, and without actual context, it won't always tell us the full story.
